The Crystal Ridge Care Center facility is a senior care center located in Grass Valley in the state of California. We offer rehabilitation, long-term care, respite care, and specialized nursing services in Nevada County.
The Crystal Ridge Care Center Mission
Since 1987, Crystal Ridge Care Center has been nestled in the picturesque Sierra Nevada mountains, offering both long and short-term care, skilled nursing, rehabilitation, and respite care in a safe and inviting environment. With 99 beds available, a highly skilled staff, and cutting-edge equipment and senior care practices, we ensure the highest standard of care for our residents.

The Crystal Ridge Care Center Team
Our skilled nurses, therapists, caregivers, and support staff work together to provide personalized, compassionate care that helps every resident achieve the highest possible quality of life. Through collaboration, clinical expertise, and a commitment to treating every resident like family, our interdisciplinary team is dedicated to supporting health, recovery, and independence.
Leadership Team
Our Licensed Nursing Home Administrator (LNHA) directs all Crystal Ridge Care Center operations, staffing, services, and programs. This leadership team also includes our Medical Director, who coordinates medical services according to policies and procedures, as well as our Director of Nursing, Rehabilitation Director, Social Services Director, and Environmental Services Director.
Nursing / Clinical Staff
Our Director of Nursing directs our nursing staff in delivering care and clinical services according to quality standards. Registered Nurses (RNs) assess, plan, provide, and document nursing care. They oversee Certified Nursing Assistants (CNAs), who assist patients with activities of daily living including administering medications, eating, bathing, dressing, and transferring.
Therapy / Rehabilitation
Our licensed Physical Therapists (PTs), Speech Therapists (STs), and Occupational Therapists (OTs) create and implement therapy plans according to the individual needs. Under the guidance of therapists, Restorative Nurse Assistants (RNAs) assist with regular activities to improve mobility and support recovery. This team is led by our Rehabilitation Director, who guides therapy and rehabilitation strategies, operations, and staff.
Social Services & Activities
Our Social Services Director leads and plans programs that address the mental, social, and emotional needs of patients and families, and oversees Social Workers. Social Workers identify and coordinate individual services to maximize independence, dignity, and quality of life. Our Activities Director plans activities for our residents.
Dietary Services
Our Registered Dietitians (RDs) develop individualized nutrition plans that support healing, recovery, and long-term wellness. Working closely with the nursing and clinical teams, they oversee meal planning, accommodate specialized dietary needs, and ensure every resident receives nutritious, satisfying meals that complement their personalized care plan.
Environmental Services
Our Environmental Services team helps create the safe, welcoming environment that Crystal Ridge Care Center is known for. From maintaining our spacious grounds and comfortable common areas to overseeing housekeeping, laundry, and facility maintenance, they work diligently to provide a clean, comfortable setting where residents and visitors feel at home every day.
